For good and valuable consideration, the receipt of which is hereby acknowledged, I, hereby grant HaifaiOne . Roller Skate Detroit permission to use my likeness in a photograph and or video footage in any and all of its publications, including but not limited to all of HaifaiOne . Roller Skate Detroit's printed and digital publications. I understand and agree that any photograph using my likeness will become property of HaifaiOne . Roller Skate Detroit and will not be returned.

I acknowledge that since my participation with HaifaiOne . Roller Skate Detroit is voluntary, I will no financial compensation.

I hearby irrevocably authorize HaifaiOne . Roller Skate Detroit to edit, alter, copy, exhibit, publish or distribute my image for purposes of publicizing HaifaiOne . Roller Skate Detroit's programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written pr electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising out of, or related to, the use of my image and likeness. I nearby hold harmless and release and forever discharge HaifaiOne . Roller Skate Detroit from claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on the behalf of my estate have or may have by reason of this authorization.

I am at least 18 years of age and am competent to contract in my own name. I have read this release before signing, e-signing and I full understand the content, meaning, and impact of this release.

In consideration of being allowed to participate in any way in HaifaiOne . Roller Skate Detroit’s programs, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while certain rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest staff member immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS ROLLER SKATE DETROIT their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Waiver of Liability and Assumption of Risk Agreement with Media Release

In consideration of being allowed to participate in any way in Roller Skate Detroit programs, related events, and activities, the undersigned acknowledges, appreciates, and agrees as follows:

Acknowledgment of Risks
I understand that the activities involved in this program carry inherent risks of injury and illness, including but not limited to communicable diseases such as MRSA, influenza, and COVID-19. These risks can be significant, and may include the possibility of permanent paralysis or death. While specific rules, protective equipment, and personal discipline may reduce these risks, I recognize that the risk of serious injury or illness still exists.

Assumption of Risks
I knowingly and freely assume all such risks, both known and unknown, even those arising from the negligence of the releasees or others. I accept full responsibility for my participation in this program.

Compliance with Safety Standards
I agree to comply with all stated and customary terms and conditions for participation. If, during my presence or participation, I observe any unusual or significant hazard, I will immediately remove myself from participation and inform the nearest staff member of the hazard.

Release of Liability
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Roller Skate Detroit, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releasees”) from any and all claims, demands, losses, or liability for injury, illness, disability, death, or damage to person or property, arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

Media Release
I grant permission to Roller Skate Detroit and its representatives to capture, use, and share photos, videos, and audio recordings of me from any events or activities in which I participate. I understand that these images and recordings may be used for promotional, marketing, and informational purposes, including but not limited to social media, website content, and other digital or print media. I waive any right to review or approve the final materials and agree that I will not receive any compensation for their use. If I choose not to be recorded, I will inform a staff member prior to the start of the event.

I HAVE READ AND UNDERSTOOD THIS RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND MEDIA RELEASE AGREEMENT. I UNDERSTAND THAT I AM WAIVING SUBSTANTIAL RIGHTS BY SIGNING IT, AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
